Commercial invoice

All documents must be signed by an authorized person or agent and notarized by a local notary public. Local Chamber of Commerce certification is not required on these documents unless used as a notary.
Provide an original and at least five (5) signed copies to be forwarded to consignee or consignee's agent.
Freight charges must be shown separately from the merchandise value.
A statement must be included that the items in the shipment have been manufactured in the (country of origin), with the full name and address of the manufacturer; or, if there are foreign parts in the items of the shipment, the country of origin must be stated and their percentage.
